I understand that failure to provide proof of insurance for my event will result in my application for rental of facilities being cancelled, and that my deposit will be lost.

By clicking below, I hereby apply to use the premises and facilities of Harvest Church for the dates and for purposes set out above. I acknowledge, confirm, and agree (i) I have read Harvest Church’s facilities use policy and the regulations regarding the use of the Premises; (ii) that my application will be evaluated in accordance with the Facilities Use Policy, and that it is a condition to my application being reviewed that I have agreed that the review of the application will be based on and governed by the Policy, and (iii) if approved, I will be required to sign a facilities use agreement, including price and additional terms.
